It is important that you bring these forms with you to the office. Please do not email them as they contain protected health information (PHI).

• Dr. Gumina: Pre- & Post-Op Instructions

In order to minimize registration time and get you with your surgeon as quickly as possible, please make sure you bring the following items with you.

• California Picture ID
• Insurance Cards
• Completed Registration Forms
• List of Medications, if Applicable
• Copay for Specialty Service as Required by Your Insurance Company

Valley Surgical Specialists accepts most major insurances. If you have specific questions, please email Vickie Arballo.

Valley Surgical Specialists accepts cash, check and credit card payments (Visa/Mastercard). Co-pay amounts vary based on type of insurance but all co-pays will be collected at time of service.

Please note: Valley Surgical Specialists collects a processing fee of $25.00 for completing all state disability, FMLA, cancer policy claim forms, and all other disability type forms. Payment is expected when the forms are submitted for completion.

If for any reason you need to cancel a scheduled appointment, please provide us 24-hour prior notice. There will be a $25 charge for any no-show or late cancellation.
